{"id":1907,"date":"2015-11-06T19:58:24","date_gmt":"2015-11-06T19:58:24","guid":{"rendered":"http:\/\/nexushc.com\/?p=1907"},"modified":"2016-07-13T18:33:42","modified_gmt":"2016-07-13T18:33:42","slug":"manager-care-coordination-rn-wa","status":"publish","type":"post","link":"http:\/\/nexushc.com\/?p=1907","title":{"rendered":"Supervisor, Case Management \u2013 RN (FL)"},"content":{"rendered":"<p><span class=\"highlight1\">Jacksonville and Fort Lauderdale area,\u00a0FL<\/span><\/p>\n<p>The Manager, Care Coordination, is responsible for the oversight, management and optimization of all care coordination activities as it relates to pre-admission, ambulatory case management, inpatient, social service, home health, health education, behavioral health and other health care delivery programs.<!--more-->Manages staff directly and is responsible for the day-to-day operations of the care management program.<\/p>\n<p><strong>Responsibilities<\/strong><\/p>\n<ul>\n<li>Manage and mentor staff to meet departmental goals; includes Case Managers, Social Workers, and Navigators.<\/li>\n<li>Manage schedules and resourcing to ensure proper coverage and staffing ratios to support the Care Coordination practice models<\/li>\n<li>Ensure the appropriate administration of member benefits<\/li>\n<li>Gather and analyze program data and implement action plans that result in\u00a0measurable outcomes.<\/li>\n<li>Manage and supports the on-going person-centered planning process.<\/li>\n<li>Monitors risk assessments for completion by Case Managers within the required\u00a0timeframe.<\/li>\n<li>Confirms that the enrollee\u2019s preferences and goals are honored.<\/li>\n<li>Ensures that that Case Managers provides the enrollee with any necessary assistance\u00a0and accommodations to prepare for and fully participate in the care planning process.<\/li>\n<li>Conducts staff meetings, assuring policy and procedures are adhered to and when\u00a0necessary, modified to address changing strategic objectives.<\/li>\n<li>Responsible for orientation of new staff, on-going training, mentoring, and\u00a0performance reviews.<\/li>\n<\/ul>\n<p><strong>Requirements<\/strong><\/p>\n<ul>\n<li>Bachelor&#8217;s degree (B.A. or B.S., BSN is preferred) from a four-year college or university plus valid\/current State of Washington RN license.<\/li>\n<li>Case Management training, certification preferred.<\/li>\n<li>Minimum of three (3) years clinical experience.<\/li>\n<li>Four (4) years of experience in an HMO\/IPA\/Managed Care setting.<\/li>\n<li>Strong leadership skills with experience as a Manager or Supervisor.<\/li>\n<li>Innovation, flexibility, and adaptability are key attributes.<\/li>\n<li>Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.<\/li>\n<li>Knowledge of, clinical standards of care, NCQA requirements, CMS guidelines, and Medicaid\/Medicare contracts and benefit systems is preferred.<\/li>\n<li>Knowledge of CPT, ICD-9 and HCPCS codes<\/li>\n<li>Basic computer skills in a Windows operating environment including Microsoft Word,\u00a0Excel, and an e-mail system.<\/li>\n<li>Strong interpersonal and decision-making skills<\/li>\n<\/ul>\n<p><a class=\"button large button default fusion-button button-3d button-round button-large button-default button-28 buttonshadow-no\" title=\"Apply Now\" href=\"mailto:nexusexecutivesearch@gmail.com?subject=I am interested in VP, Medical Management \u2013 RN (NB)\" target=\"_blank\" type=\"button\"><span class=\"fusion-button-text\">Apply Now<\/span><\/a><\/p>\n<p><span style=\"color: #336699;
